This survey will ask you questions about your wellbeing. Wellbeing includes both your physical and mental health. Your teacher or teaching assistant will help you complete the survey if you need help.

The next few questions are about how you see yourself as a person. For each item decide whether it describes you. It would help us if you answered all items as best you can even if you are not completely sure. 

'Not True' means that this does not describe you as a person at all.

'Somewhat True' means that this does describe you some of the time.

'Certainly True' means that this definitely describes you as a person.
